Montreal Heart Institute Announces Expansion of ý Digital Care Journeys for TAVI, TMVR, Other Mitral and Tricuspid Procedures

Remote Patient Monitoring Reduces ED visits and Readmissions for Transcatheter Aortic Valve Replacement Procedures

Montreal and Toronto, CanadaFebruary 7, 2023 – , the flagship of cardiovascular medicine in Quebec, a recognized research leader in Canada and around the world, and a true spearhead in cutting-edge cardiovascular medicine today announced the expansion of , a leading Digital Care Journey platform, to their Transcatheter Aortic Valve Implantation (TAVI), Transcatheter Mitral Valve Replacement (TMVR) and other mitral and tricuspid procedures. The Institute first launched ý Digital Care Journeys for Open Heart Surgery to optimize patient engagement and operational efficiencies. Building upon their digital patient engagement strategy, now patients undergoing TAVI, TMVR, Mitral and Tricuspid procedures can access ý to be guided step-by-step from pre-surgical preparation through post-surgical recovery via personalized evidence-based education, progress-tracking, and post-op symptom monitoring using their own smart device or computer. The program is available in English and French.

“Introducing ý into our TAVI program enabled us to reduce 30-day ED visits by 50.1% and reduce 30-day hospital readmissions by 33.5% among the patients using the application,” said Dr. Walid Ben Ali, Co-Director of the Transcatheter Valve Therapy Program at The Montreal Heart Institute, whose team recently . “Deploying a digital patient engagement solution optimized the clinical trajectory, achieving a median readmission rate lower than the Canadian national rate (7.9% vs 9.4%). The use of digital applications like ý is an effective way to remotely monitor patients, digitally collect Patient-Reported Outcomes (PROs), and track symptoms in real-time.”

With the ý platform, The Montreal Heart Institute can engage, connect and remotely monitor patients by delivering consistent evidence-based education in an interactive and patient-friendly way that enables patients to actively participate in their own care before, during, and after cardiac procedures. While recovering at home, patients can self-report data such as pain scores, symptoms, and share incision photos through the platform. Meanwhile, providers can monitor patients on dashboards allowing for personalized care recommendations and faster intervention from the surgical care team.
