[0:00] Introducing Ms. Daniela Faykoo Martinez, a registered Dietician and currently a Sr. Patient Education Specialist at ������ý where she designs surgical digital care plans for patients. Daniela has a Bachelor of Applied Science in Nutrition and Food and a Masters of Health Science in Nutrition Communication from Ryerson University.

[1:30] How Digital Patient Engagement works for patients and providers by engaging & monitoring patients through their surgical journey via smartphone, tablet or desktop computer. Patients receive reminders, interactive education and are empowered to track their progress;

[3:27] How transcatheter aortic valve replacement/implantation (TAVR/TAVI) is a procedure performed as a safer alternative to open heart surgery for high risk patients. Patients experience severe aortic stenosis and require a prosthetic valve put into their diseased valves to help the blood flow;

[4:35] How TAVR programs are seeing a lot of growth recently as a safer option than open heart surgery as its minimally invasive with a quicker recovery timeline;

[5:36] Why TAVR patients are mainly concerned about the pain they might experience as the incision sites are often sensitive areas (e.g. neck, groin, chest). They are also concerned about understanding the details of the procedure, and the prosthetic items;

[6:55] How the ������ý care plan caters to a wide variety of learning styles whether it be providing text for patients who prefer reading, providing illustrations of the anatomy for visual learners, as well as having videos of the procedure;

[8:05] How Ms. Martinez leveraged her clinical perspective to understand from the patient’s perspective what specific features she would need to build into the technology. One of the most important features she highlighted is ensuring patients can monitor their vitals once they get home;

[10:43] How TAVR patients receive custom recommendations on self-management based on their input of symptoms into the app, (e.g. baseline blood pressure, device erosion, signs of stroke, delirium, loss of coordination), and how the most engagement is within the first 2 weeks;

[13:05] How patient feedback indicates that patients enjoy the ability to monitor vitals and avoid phone calls for more minor issues, and generally feel empowered to self-manage;

[19:00] How TAVR patients usually undergo multiple procedures at the same time (e.g. also receiving a pacemaker), and how symptom monitoring is usually similar between the procedures (e.g. endocarditis, device erosion, delirium, etc.);

[20:00] Why it’s important to explain to patients who is on their care team, who they should expect to talk to, and what happens during the surgery;
