The Inaugural Seamless Care Awards

We are excited to announce the launch of our first-ever “Seamless Care Awards”, a program designed to celebrate and recognize the exceptional achievements of our healthcare partners. These awards spotlight the remarkable efforts our partners put into enhancing patient care through digital care journey innovation.

Award Categories and Criteria

To select the finalists and award recipients, the ý Customer Success team did a thorough review of program and engagement data, customer stories, etc. and meaningful debate. It was incredible to reflect on how innovative our partners have been with digital health.

The Seamless Care Awards are divided into three categories, each reflecting a core area of excellence in healthcare delivery:

  1. Patient Engagement:
    • Patient Advocacy Award: Honoring the health system with the highest patient adoption of digital care journeys.
    • Finalists: Grand River Hospital, Bluewater Health, Brant Community Healthcare System

      Winner: Brant Community Healthcare System

    • Active Patient Participation Award: Recognizing the hospital with the highest level of patient engagement.
    • Finalists: Southlake Regional Health Centre, Dr. Freidrich Boettner, Home and Community Care Support Services – Central East

      Winner: Home and Community Care Support Services – Central East

    • Excellence in Patient Experience Award: Awarded to the health system that achieves the highest patient satisfaction ratings with ý.
    • Finalists: Trillium Health Partners, Atrium Health, Thunder Bay Regional Surgical Services

      Winner: Trillium Health Partners

  2. Change Management:
    • Lightning Transformation Award: For the fastest implementation of ý.
    • Finalists: Southlake Regional Health Centre, Thunder Bay Regional Surgical Services, Muskoka Algonquin Healthcare

      Winner: Southlake Regional Health Centre

    • Change Agent of the Year Award: Celebrating the health system that has implemented the most ý care plans in the past year.
    • Finalists: Sault Area Hospital, Niagara Health, Horizon Health Network

      Winner: Sault Area Hospital

    • Brand Ambassador Award: Awarded to the health system with the most innovative use of marketing and communications for their digital care journey strategy.
    • Finalists: Halton Healthcare, Oak Valley Health, Canadian Mental Health Association: Fort Frances Branch

      Winner: Oak Valley Health

  3. Innovation, Quality, and Research:
    • Research Trailblazer Award: For the health system that has published and presented the most research about ý.
    • Finalists: Baystate Health, Ohio State University Wexner Medical Center, UAB Medicine

    • Impactful Improvement Award: Recognizing the health system with the biggest improvement in patient outcomes.
